Career After MBBS from Russia: FMGE, Internship, PG and Specialist Routes

A degree from a Russian university is the start of the medical career, not the end of it. In India it leads through FMGE, a year of internship and registration to NEET-PG, and from there to MD, MS or DM. Abroad it leads to USMLE, PLAB or AMC. This guide lays out each step with the years it takes, what the exams cost and what a doctor in Kerala is paid.

In short: after MBBS in Russia you pass FMGE, do a 12-month supervised internship in India, register with the state medical council (the Travancore-Cochin Medical Council in Kerala) and then sit NEET-PG for an MD or MS seat. A specialist such as a cardiologist or a neurologist needs a further super-speciality course (DM) after an MD. From a Russian MBBS the route to a cardiologist is therefore about 6 years of MBBS, 1 year of internship in India, 3 years of MD and 3 years of DM, plus the time you need to pass FMGE and NEET-PG. USMLE, PLAB and AMC are the options for working abroad.

2. FMGENBEMS screening test, 150 of 300 marks to pass, three sessions a yearMonths; no limit on attempts
3. Internship in India12-month supervised internship in an Indian medical college1 year
4. RegistrationPermanent registration with a state medical councilWeeks after internship
5. NEET-PGEntrance for MD, MS and PG diploma seatsExam once a year; MD/MS course 3 years
6. NEET-SSEntrance for DM and MCh super-speciality seats, after an MD, MS or DNBCourse about 3 years
Steps and conditions from the NMC rules for foreign medical graduates (FMGL 2021 and the NMC FAQ of 8 June 2026) and the NBEMS bulletins for FMGE, NEET-PG and NEET-SS. Registration conditions are decided by the state medical council.

The first two steps are what separate a Russian graduate from an Indian one. Everything after them is the same system, the same exams and the same seats. A Russian MBBS holder who has passed FMGE and finished the Indian internship competes for PG seats on the NEET-PG rank like anyone else. For the full rules on the degree itself, see the NMC rules for foreign medical graduates.

FMGE: the first hurdle

Every Indian citizen with a foreign MBBS has to qualify FMGE before internship and registration. NBEMS now holds the exam three times a year; the October 2026 session is on 31 October. The exam has 300 multiple-choice questions, no negative marking, and a pass mark of 150. Details, fee and Kerala centres are on the FMGE exam page. Pass rates for Russian graduates were 25.8% in 2025, and each university's figure is on the FMGE pass rate page. NExT, the exit test that is meant to replace FMGE, has no start date. Read FMGE vs NExT for what that means for a 2027 student.

Internship in Kerala after FMGE

The NMC requires a 12-month internship in the country of study and then a 12-month supervised internship in India. The second one is the CRMI (compulsory rotating medical internship). It is allotted through the state medical council, and for 2026-27 the NMC slot matrix lists 1,607 internship slots for foreign graduates in Kerala. Students from Kerala usually want to do it in Kerala, so apply early; slots are limited and depend on the order and documents. The conditions the council checks include the 54-month course in English, the foreign internship at the same university, the licence to practise in Russia and the FMGE pass certificate. We explain these in the guide to MBBS internship rules from the NMC.

The internship is paid in some places and not in others. We could not find a current official stipend figure for foreign graduates in Kerala, so ask the hospital in writing before you accept a posting.

PG in India: NEET-PG, MD and MS

Once you hold a provisional or permanent registration and have completed the internship, you can sit NEET-PG, the entrance test conducted by NBEMS. The NBEMS page on NEET-PG lists the basic condition: an MBBS degree, an NMC or state registration, and one year of internship completed by the cut-off date for the session. For NEET-PG 2026, held on 30 August 2026, NBEMS fixed 30 September 2026 as the date by which the internship had to be completed, as reported by Careers360. Foreign graduates also need the FMGE pass certificate at the centre. The 2027 exam date is not notified yet.

How long and how many seats

MD and MS in a broad speciality take three years. A press report on the NMC seat matrix for NEET-PG counselling 2026 puts the total at more than 50,000 PG seats, though the exact number changes with each round as new seats are added. Seats are in government colleges, private colleges and deemed universities, and the fees differ enormously.

What a PG seat costs

SeatFee per yearBasis
Kerala government medical college, MD/MS₹57,890 tuition (₹11,580 miscellaneous, ₹11,580 refundable deposit)Kerala G.O.(Ms) No. 92/2023/H&FWD; check whether it still applies to your batch
Private deemed university, clinical branches₹9 lakh to ₹76 lakhCareers360 analysis of about 720 courses at 44 deemed-university colleges, 2025
Most expensive: radiodiagnosis, Santosh Medical College, Ghaziabad₹76 lakh (₹2.28 crore for the course)Same analysis
In the same analysis, 313 of about 720 courses cost more than ₹1 crore for the three years, and at least 15 programmes at eight institutes cost more than ₹2 crore. We found no official self-financing PG fee list for Kerala for 2026-27; see cee.kerala.gov.in when the PG prospectus is out.

The difference is the reason a good NEET-PG rank matters more than anything else after MBBS. A government seat costs a few thousand rupees a month, and a private deemed seat can cost more than the entire six years of MBBS in Russia. Compare it with the MBBS fees in Kerala vs Russia before you assume that a PG seat can be bought later.

How to become a cardiologist, neurologist, surgeon or other specialist

These are the usual routes in India. The years are the course lengths; they do not include the time you take to pass FMGE or to prepare for NEET-PG and NEET-SS.

SpecialistRoute after MBBSYears of PG
CardiologistMD General Medicine (NEET-PG), then DM Cardiology (NEET-SS)3 + 3 = 6
NeurologistMD General Medicine (or paediatrics), then DM Neurology (NEET-SS)3 + 3 = 6
General surgeonMS General Surgery (NEET-PG)3
Heart surgeon (cardiothoracic)MS General Surgery, then MCh Cardiothoracic and Vascular Surgery (NEET-SS)3 + 3 = 6
DermatologistMD Dermatology, Venereology and Leprosy (NEET-PG)3
GynaecologistMS or MD Obstetrics and Gynaecology (NEET-PG)3
PaediatricianMD Paediatrics (NEET-PG)3
Routes follow the NMC postgraduate system: broad-speciality MD/MS through NEET-PG, super-speciality DM/MCh through NEET-SS. DM and MCh length is taken from course listings of about 36 months; check the NMC PG regulations for each course. The institutes AIIMS, PGIMER Chandigarh, JIPMER, NIMHANS and SCTIMST admit to DM/MCh through INI-CET instead of NEET-SS.

So the answer to "how to become a cardiologist after 12th" is long: MBBS, internship, MD Medicine, DM Cardiology. From Russia the count runs about 6 years of MBBS, 1 year of internship in India, 3 + 3 years of PG, so about 13 years after Class 12 if you pass FMGE and entrance exams on the first try. How many years to become a surgeon? About 10 (6 + 1 + 3). A DM or MCh seat is far harder to get than an MD seat. NEET-SS needs an MD, MS or DNB first, and NBEMS sets the cut-off date by which that degree must be done. Dates move from year to year, so read the bulletin of the year you apply.

A DNB (Diplomate of National Board) is another path. Many large private hospitals in Kerala are accredited for DNB training, with admission through NEET-PG counselling. This page does not give DNB fees because they are set hospital by hospital.

Practising abroad after MBBS from Russia: USMLE, PLAB, AMC

You can also work outside India. Each country has its own licensing route, and a Russian degree is judged by the country's own rules, not by the NMC.

  • United States (USMLE and ECFMG). You pass Step 1 and Step 2 CK, apply for ECFMG certification, and then match to a residency. The 2026 USMLE fee is $695 for Step 1 and for Step 2 CK, and $955 for Step 3 (about ₹66,000, ₹66,000 and ₹91,000 at 95.03 rupees per dollar), plus a surcharge for testing outside the US. ECFMG's application fee has been $580 since April 2026. Since 2024 ECFMG has required that applicants come from a medical school accredited by an agency recognised by the World Federation for Medical Education. Not every Russian university meets that, so check your university on ECFMG's list before you pay for anything.
  • United Kingdom (PLAB). PLAB 1 and PLAB 2 are the GMC's tests, with an English test (IELTS or OET) first. The fees quoted from the GMC's table from April 2026 are £283 for PLAB 1 and £1,036 for PLAB 2; confirm them on gmc-uk.org. The GMC also checks that your medical school is listed in the World Directory of Medical Schools and that you completed an internship in the country of study, which a Russian graduate normally has.
  • Australia (AMC). The standard pathway for international graduates is the AMC CAT MCQ examination followed by the AMC clinical examination or workplace-based assessment, after the AMC checks your degree. The Australian authorities set their own rules on which schools are acceptable.

These are separate from the Indian route. A student who wants both options should choose a university that is well recognised internationally and check the list of the target country on the first day, not after graduation. Most Kerala families who ask us about USMLE or PLAB eventually register in India first, because the internship and FMGE are mandatory for an Indian licence.

PG in Russia?

Russian universities also offer postgraduate residency (ordinatura). Teaching is in Russian, and a Russian PG degree does not replace the Indian one. Very few Indian students choose it, and the ones who do usually plan a career outside India. If you consider it, ask the NMC and the target country how they treat the qualification before you apply.

What does a doctor earn in Kerala?

Salary is the question parents ask most, and there is no honest single number. What we can show comes from official and press sources.

  • Government service. The Kerala Public Service Commission notification for the Assistant Surgeon or Casualty Medical Officer in the Health Services Department (category 177/2025) gives a pay scale of ₹63,700 to ₹1,23,700 a month. The post asks for an MBBS degree and valid registration with the Travancore-Cochin Medical Council, so a Russian graduate qualifies after FMGE, internship and registration.
  • Junior doctors in hospitals. The Indian Medical Association Kerala said in June 2026 that a Government Medical College had offered ₹42,000 a month for MBBS doctors as casualty medical officers, and demanded a minimum of ₹80,000 a month for junior doctors (Medical Dialogues, 29 June 2026).
  • After PG. Pay rises with the specialty, the hospital and private practice, and we do not quote a figure for it because we found no reliable public source.

More on pay is in doctor salary after MBBS from Russia. The practical conclusion is that a young doctor's income in the first few years is modest, and the return on an MBBS comes through PG and the career afterwards. That is also why loans should be sized for the course, not for an imagined salary; see the education loan guide.

What helps a Russian graduate on this road

  • Choose by FMGE record. Everything after the degree waits for the pass certificate. See the ranking of Russian universities by FMGE results.
  • Start FMGE and NEET-PG revision during the course. The syllabus is the Indian MBBS syllabus; the clinical subjects are 200 of the 300 FMGE marks.
  • Keep documents in order. The apostilled diploma, internship certificate from the Russian university, NEET result, passport and Class 12 papers are asked for at the FMGE, at the internship and at registration. The first step for Kerala certificates is described in the NORKA attestation guide.
  • Plan the money for the extra years. A PG seat in a government college is cheap; a private one is not. Treat a government seat as the goal and a private seat as a decision for the family to make with the numbers in front of it.
