MBBS in Germany for Indian Students: Fees, German Language and How It Compares with Russia

Germany has no MBBS and no easy way in for a Kerala student after Class 12. Public universities charge no tuition, which is why the idea spreads, but medicine is taught in German, places are rationed and the cost of living is high. This page sets out the real requirements from DAAD, the German missions in India and university sites, and compares them with Russia.

In short: public universities in Germany generally charge no tuition for medicine, apart from the Baden-Württemberg fee of €1,500 a semester for non-EU students, plus a semester contribution of €70–430. But the course is taught in German, Class 12 alone usually does not qualify for entry, non-EU applicants compete for a small number of places, and the visa needs €11,904 in a blocked account (about ₹12.7 lakh). Living costs are €900–1,200 a month. For most Kerala students after Class 12, Germany is a path of 8 or more years, not a cheaper MBBS.

There is no MBBS in Germany

Germany trains doctors in a single six-year course of "Humanmedizin" that ends with a state examination, not an MBBS. The federal regulation (the Approbationsordnung) sets the standard length at six years and three months including the final exam; the last year is a practical year of 48 weeks in hospitals. The graduate then applies for the "Approbation", the licence to practise. The degree is taught in German at public universities, with oral exams in German.

So searches such as "MBBS in Germany" or "MBBS degree in Germany" lead to a different thing: a German state-exam course that qualifies a doctor to work in Germany. That matters because of the NMC: its rules for foreign medical graduates need at least 54 months of study in English. A German-taught degree does not fit that rule, and the NBEMS report on the FMGE of 2025 lists no Germany in either its June or its December country tables, which means Indian graduates of German courses do not sit FMGE in any visible number. We did not find an NMC statement on German degrees, so if a return to India is part of the plan, ask the NMC before you start. The usual plan of Indian students in Germany is to work there, in German hospitals.

Tuition fees: what you really pay

DAAD, the German academic exchange service, states that public universities in most German states charge no tuition for Bachelor's courses. The exceptions that matter to a medical applicant:

  • Baden-Württemberg: non-EU students pay €1,500 a semester, including in state-examination courses such as medicine. For 12 semesters that is €18,000, about ₹19.1 lakh at ₹106.32 per euro (Central Bank of Russia rate of 15 October 2026). Doctoral students are exempt.
  • Bavaria: DAAD notes that non-EU students may face fees of several thousand euros a semester set by each institution. Check the university you apply to.
  • Semester contribution: every student pays €70 to €430 per semester for administration, student services and, in most cases, a public-transport ticket. The University of Cologne's total for winter 2024/25 was €304.25.

The tuition saving is real: a Russian state university costs about ₹14–33 lakh in tuition for six years outside Moscow, and a German public university in most states costs none. The savings disappear in living costs.

Cost of living and the blocked account

DAAD estimates a student's monthly costs at €900 to €1,200 depending on the city. The 22nd social survey of the German student service found an average of €876 a month in 2023. At ₹106.32 per euro, €900–1,200 is ₹0.96–1.28 lakh a month, or ₹11.5–15.3 lakh a year. Over the six years of the medical course, living costs alone come to about ₹69–92 lakh before any pre-study year. Compare this with the Russian student's cost of living in our cost of living guide.

For the student visa a German mission in India wants proof of funds for the first year. The German missions in India give the figure as €11,904 in a blocked account (Sperrkonto), with a confirmation that no more than €992 a month can be withdrawn; the monthly sum took effect on 1 September 2024, and DAAD lists the same €992 a month; we found no later change. At the rate above, the deposit is about ₹12.7 lakh and the monthly limit about ₹1.05 lakh. Banks charge €70–150 to open the account. A formal sponsor declaration (Verpflichtungserklärung) from a person in Germany or a German or EU scholarship can replace the account; an Indian affidavit of support cannot. The amount is revised periodically, so read the current figure on the website of the German mission before you pay.

Getting in: school certificate, German and selection

Is Class 12 enough?

Usually not. Germany rates foreign school certificates in the anabin database. The Heinrich Heine University Düsseldorf's page for non-European applicants to medicine says to check whether your certificate is recognised, and that a Studienkolleg (a one-year preparatory college ending in an assessment exam) may be required before admission. Agent sites say an Indian Class 12 certificate on its own gives access only to a Studienkolleg, or to a subject-restricted entry after one year at an Indian university. They also report that a minimum of 70% in Class 12 has applied from the winter semester 2026/27; we could not verify this on an official page, so check it with DAAD or the university.

The University of Cologne states that applicants from India must hold an APS certificate (the Academic Evaluation Centre of the German Embassy in New Delhi) and a valid TestAS certificate, including the medicine module, and must apply through uni-assist.

German language

Medicine is taught in German. Cologne accepts TestDaF with level 4 in all parts, the DSH-2 examination or telc C1 Hochschule. C1 is an advanced level; by our estimate a beginner needs well over a year of full-time study to reach it. The Studienkolleg itself is mostly taught in German, according to agent guides.

Selection

Medicine is an admission-restricted subject (numerus clausus). For students with a German or EU school certificate the places are allotted in a central procedure by grades, an aptitude test and each university's own selection. Non-EU applicants are in a separate, small group: for example, Leipzig's medical faculty lists "foreign applicants" among the advance quotas. Agent guides put it at about 5% of places, but we could not confirm the percentage on an official page. NEET does not enter the German system at all. It is an Indian requirement for going abroad for MBBS and does not substitute for any German test.

A realistic timeline from Class 12, by our estimate: a year or more to prepare German and the APS certificate, a year at a Studienkolleg or a university year in India, and then the six-year course. That is eight or more years before the Approbation, against six years of study in Russia plus the Indian route.

How Germany ranks, and what that does for you

German medical faculties rank well. In the QS World University Rankings by Subject 2026 (Medicine), Charité – Universitätsmedizin Berlin is 18th and Heidelberg 34th. In the THE 2026 Clinical and Health table Charité is 29th. A ranking does not decide whether a graduate can practise in India; the NMC rules do. Our page on the best medical universities in the world explains this.

Our verdict

Germany suits a student who wants to build a medical career in Germany or Europe, is ready to spend two years on German, an Indian certificate check and a preparatory year, and has ₹70–90 lakh or more for living costs over the course. It is a better fit for a strong student with time than for a family that needs a doctor's degree usable in Kerala in about six years.

Russia suits the student who has the NEET qualification now, who wants teaching in English, a fixed fee set against a public price list and the Indian licence route through FMGE. It does not suit a student who wants to practise in Germany. Both routes are long, and neither is "free". If the plan is Kerala, start with Kerala seats against Russia and the admission process. If you are still deciding between countries, see MBBS abroad for Kerala students.

MBBS in Germany for Indian Students: Frequently Asked Questions

Is MBBS in Germany free for Indian students?

There is generally no tuition at public universities in most German states, but not everywhere: Baden-Württemberg charges non-EU students €1,500 a semester. Every student pays a semester contribution of €70–430, and living costs are €900–1,200 a month. So the course is not free in practice.

Can I study MBBS in Germany in English?

Medicine at German public universities is taught in German, and universities such as Cologne ask for German at C1 level (TestDaF 4 in all parts, DSH-2 or telc C1 Hochschule). A programme advertised in English needs its own check against the NMC rules.

How much money is needed for a German student visa?

The German missions in India give €11,904 in a blocked account, with a withdrawal limit of €992 a month, about ₹12.7 lakh at ₹106.32 per euro. A sponsor declaration or a German or EU scholarship can replace it. Check the current figure before you open the account.

Can I go directly to Germany after Class 12 for medicine?

Usually not. Indian Class 12 certificates are normally rated as giving access only to a Studienkolleg or, after a year at an Indian university, to subject-restricted entry. Universities such as Cologne also want an APS certificate and a TestAS result.

Is a German medical degree valid in India?

The NMC rules need at least 54 months of study in English, and German courses are taught in German. NBEMS lists no Germany line in its 2025 FMGE country tables. We found no NMC statement on German degrees, so ask the NMC if you intend to return.

Is Germany cheaper than Russia for medicine?

In tuition, yes: none in most states against about ₹14–33 lakh at Russian state universities. In total cost, usually no: living costs of €900–1,200 a month, a blocked account and two or more pre-study years bring the total above Russia's for most families.
