MBBS in Russia vs MBBS in India: Fees, Seats and Quality Compared

India has 1,29,026 MBBS seats for 2025-26 and about 12 lakh students qualify NEET every year, so most qualified students never get a seat. This post puts MBBS fees in India next to MBBS in Russia and says plainly who should wait, who should go, and what the Russian route adds.

In short: MBBS in India is cheaper and quicker to a licence if you can get a government seat, because tuition is nominal (about ₹23,000 a year in a Kerala government college) and there is no extra licensing exam. There are 1,29,026 MBBS seats in India for 2025-26 against 12.37 lakh candidates who qualified NEET-UG 2025, so about nine in ten qualified students cannot get one. MBBS in Russia costs roughly ₹14–33 lakh in tuition for six years at most state universities outside Moscow, but adds FMGE and a year of internship in India. Choose Russia when the Indian options are a very expensive private seat or no seat at all.

MBBS seats in India: how many there are

According to the final NMC seat matrix (as on 3 December 2025), India has 1,29,026 MBBS seats for 2025-26. That is 11,276 more than the 1,17,750 seats of the year before, a net rise after 456 seats were removed. The figure includes AIIMS, other institutes of national importance and JIPMER. Karnataka, Uttar Pradesh, Tamil Nadu and Maharashtra have the largest numbers. Kerala has 5,404 MBBS seats for the same year, according to the NMC circular of 10 March 2026.

Now the other side. NEET-UG 2025 had 22,09,318 candidates who appeared and 12,36,531 who qualified. Setting qualified candidates against seats gives about 9.6 qualified students for every seat in India. In Kerala the ratio is worse: 73,328 qualified candidates against 5,404 seats, about 14 for every seat. NEET-UG 2026 was held again on 21 June 2026 after the 3 May paper was cancelled following a leak; about 19.99 lakh candidates appeared and 11.21 lakh qualified. That is still more than eight qualified students per seat if the number of seats stays near the 2025-26 level.

Many of those 12 lakh qualified students are not aiming for MBBS only; some want BDS, AYUSH or nursing. But even if one in three wants MBBS, seats are fewer than candidates. A small share of seats is reserved for AIIMS-type institutes and for categories, so your realistic chance depends on your rank and category.

MBBS fees in India: government, private and NRI seats

Fees in India depend on the kind of college and the state. In a government medical college the tuition is nominal. In Kerala it is about ₹23,150 a year. In a private college, fees are set by the state's fee regulatory authority. For Kerala self-financing colleges the 2025-26 provisional fee at one college was ₹8,10,175 a year for a general seat and ₹22,74,006 for an NRI seat, according to the CEE Kerala notice. Over 4.5 years that is roughly ₹36 lakh for a general seat and ₹1 crore for an NRI seat; the full workings are on our Kerala vs Russia fee comparison.

In February 2022 the NMC directed that fees for half of the seats in private medical colleges be kept on a par with government colleges of the same state. Press reports say states have applied this unevenly, so check your state's notification instead of trusting an agent's "management quota" quote. We have not verified private-college fee levels in other states for this page; use each state counselling authority's notice for the year.

MBBS in Russia fees

Russian state medical universities charge tuition in roubles. The first-year fees we have confirmed from official price lists run from ₹2.4 lakh to ₹14.1 lakh a year, with a median of ₹4.4 lakh. Moscow is at the top; most universities outside Moscow charge ₹3–5.5 lakh a year. A few examples from our university pages:

UniversityFirst yearSix years, tuition
Kursk State Medical University₹2.7 lakhabout ₹16 lakh
Orenburg State Medical University₹4.8 lakh₹22.1 lakh
Bashkir State Medical University₹5.5 lakhabout ₹33 lakh

Six-year totals marked "about" assume the first-year fee stays the same; keep a reserve of 10–20% because fees usually rise. Years to a licence: Russia vs India

An Indian MBBS is a 4.5-year course followed by a one-year compulsory rotating internship, so a student who joins in 2026 can be registered in about 5.5 years. A Russian degree takes six years, including the clinical internship the NMC requires at the same university. The graduate then has to pass FMGE and complete a further 12 months of supervised internship in India before full registration. Counting a few months for FMGE preparation, the Russian route usually takes seven years or more from joining to registration. The NMC also requires the foreign course to be completed within 10 years of joining.

Quality of education: what is the same and what is not

The NMC requires a foreign course to teach the same subjects as the Indian MBBS: anatomy, physiology and biochemistry first, then pathology, pharmacology and microbiology, then medicine, surgery, obstetrics and the other clinical branches. Russian state medical universities have taught medicine for decades; the oldest in our list were founded in the 18th and 19th centuries. See the MBBS subjects in Russia post for the year-by-year plan.

Three things differ. First, the language of the wards: lectures for Indian groups are in English, but patients speak Russian, so a student who does not learn Russian gets less from the clinical years. Second, the patients and the disease pattern are those of Russia, while FMGE follows the Indian syllabus, so students must keep Indian practice in mind while studying abroad. Third, FMGE: graduates of Russian universities sat the exam 11,428 times in 2025 and 25.8% passed. The universities differ widely, so read the FMGE pass rate by university before choosing.

Who should choose India and who should choose Russia

Wait for or take an Indian seat if

  • your rank is close to the last government seat ranks of any state you can apply to, including All India Quota seats outside Kerala;
  • your family can afford a private seat without borrowing heavily;
  • you cannot imagine six years away from home.

Consider Russia if

  • you have qualified NEET but no government seat is realistic and the Indian private or NRI fee is beyond the family's means;
  • you are ready to study for FMGE from the third year, not after graduation;
  • you will learn Russian enough to speak to patients.

A repeat year for NEET is a third option and a good one for a student who missed a seat by a small margin. The decision for each family is different, so we compare your own numbers: see how many years MBBS in Russia takes, the admission steps and the ranking of universities by FMGE results.
