How to Become a Gynecologist: MBBS, NEET-PG and MS/MD OBG, with Years and Costs

A gynecologist in India is an MBBS doctor with a postgraduate qualification in Obstetrics and Gynaecology (OBG): an MS or MD, a DNB, or a two-year diploma. This guide gives the route from Class 12, the minimum years, the cost of three typical routes for a Kerala family, and the seat numbers you will compete for.

In short: to become a gynecologist in India you need Class 12 with Physics, Chemistry and Biology, NEET-UG, an MBBS with a year of internship, and then a postgraduate seat in Obstetrics and Gynaecology through NEET-PG: MS or MD for three years, or DNB for three years. The minimum is 8.5 years after Class 12 with an Indian MBBS and 10 years with a Russian MBBS (6 years in Russia, 12 months of internship in India after FMGE, 3 years of PG). The NMC seat matrix for 2026-27 lists 4,690 MS OBG seats in India, 142 of them in Kerala.

What a gynecologist does, and what the qualification is called

"Gynecologist" is the everyday word. The qualification is Obstetrics and Gynaecology, written OBG or OB-GYN. Obstetrics is care of a woman during pregnancy, labour and the weeks after birth. Gynaecology covers the female reproductive system at every age: periods, infertility, fibroids, infections, menopause and cancer screening. One specialist does both, and also operates, including caesarean sections and hysterectomies, so OBG is part clinical medicine and part surgery.

The work is shift work. A resident in a government hospital takes labour-room and emergency duty at night. That is a fact to tell a student before the choice, and it affects family life later. OBG is open to women and men; some patients and families prefer a woman doctor, but that is a preference and not a rule.

Three ways to qualify in OBG after MBBS

All three start with the NEET-PG entrance and MBBS registration. The difference is the length, the title and how easy the seat is.

QualificationLengthWhat to know
MS or MD in Obstetrics and Gynaecology3 yearsThe standard degree, in medical college hospitals. 4,690 MS OBG seats in India in 2026-27.
DNB in Obstetrics and Gynaecology3 yearsAwarded by the National Board of Examinations, mostly in large hospitals, entered through NEET-PG counselling. The NHM Kollam notice below accepts a DNB in OBG.
Diploma in Obstetrics and Gynaecology (DGO)2 yearsA shorter qualification. Under the NMC PG regulations 2023, as summarised by Medical Dialogues, colleges can convert diploma seats to degree seats and no new diploma seats are being taken up, so the number is shrinking.
Sources: NMC PG Medical Education Regulations 2023 as summarised by Medical Dialogues (three years for MD/MS, two for diploma); NMC seat matrix 2026-27 as reported by Medical Dialogues; NHM Kollam notice No. NHM/KLM/133/2026-A3 of 20 August 2026.

A doctor who already holds a recognised two-year diploma in the same subject can finish the MD or MS in two years, according to the same regulations. After the degree, some doctors take a fellowship in a narrower area such as reproductive medicine or high-risk pregnancy. Fellowships are run by hospitals and academic bodies, their length and recognition differ, and a certificate is not always a recognised qualification, so check recognition before paying. A formal super-specialty also exists: MCh in Gynaecological Oncology through NEET-SS; the final NEET-SS 2025 matrix listed 24 DrNB seats in that subject.

From Class 12 to a gynecologist: the steps

A short version of the road, with the year each step usually ends.

  • Class 12 (PCB) and NEET-UG. Age 17 by 31 December, 50% in PCB (40% for reserved categories). NEET-UG 2026 qualifying mark: 213/720 for general and EWS (144 in 2025), 194 for general PwBD, 177 for SC, ST and OBC. The 3 May 2026 exam was cancelled after a paper leak and re-held on 21 June; the result came on 16 July. See NEET 2026 result: what next.
  • MBBS. India: 4.5 years plus a 12-month internship. Russia: 6 years, meaning at least 54 months of English-medium study and a 12-month internship at the same university under the NMC rules of 2021.
  • FMGE, for degrees from abroad. Held three times a year since 2026; about one in four Russian graduates passed over 2022–2025. See the FMGE exam guide.
  • Internship in India, 12 months. Required again for graduates from abroad.
  • NEET-PG. NEET-PG 2026 was held on 30 August; the result came on 24 September with a qualifying score of 262 for general and EWS candidates. A graduate from abroad must have passed FMGE and finished the internship by the cut-off in the NBEMS bulletin.
  • MS or MD OBG, 3 years, then registration as a specialist.

The minimum is 4.5 + 1 + 3 = 8.5 years with an Indian MBBS, and 6 + 1 + 3 = 10 years with a Russian MBBS, counted from the start of MBBS and without a single repeat year. A student who joins MBBS at 18 can be a gynecologist at 26 or 27 on the Indian route and at 28 on the Russian route. Plan for longer, because NEET-PG is held once a year and counselling takes months.

What the three typical routes cost

These are tuition figures, from official documents or university price lists, for three routes a Kerala family may be comparing. Hostel, food, travel and coaching are not included. Russian costs are for universities outside Moscow.

RouteMBBS tuitionPG tuitionTotal tuition
A. Kerala government MBBS, then a Kerala government PG seat₹23,150 a year; about ₹1 lakh₹57,890 a year; about ₹1.7 lakh for three yearsabout ₹2.8 lakh
B. Kerala self-financing MBBS, then a Kerala government PG seat₹8,10,175 a year (2025-26, provisional); about ₹36 lakhabout ₹1.7 lakhabout ₹38 lakh
C. Russian state university, then a Kerala government PG seat₹2.4 lakh to ₹14.1 lakh a year; about ₹14–33 lakh for six yearsabout ₹1.7 lakhabout ₹16–35 lakh, plus living costs abroad
Sources: Kerala government MBBS tuition and CEE Kerala MBBS notification for 2025 (self-financing); CEE Kerala PG medical prospectus 2023-24 (₹57,890 annual tuition, ₹11,580 miscellaneous fee); Russian price lists, see the fees page. A self-financing PG seat is not counted here: the 2025 CEE Kerala list as reported by Medical Dialogues shows ₹18,36,456 to ₹19,42,235 for a clinical seat, which would be ₹55–58 lakh for three years if charged yearly.

Route A depends on a rank very few students achieve. Route C looks cheaper than route B on tuition, but adds living costs abroad, FMGE preparation and a longer road. Route C also needs a PG seat: a graduate from abroad competes in NEET-PG on the same terms. Kerala government PG seats are meant for candidates of Kerala origin and carry a bond: one year of Senior Resident service after the course and ₹50 lakh as liquidated damages if you leave after the final allotment (CEE Kerala prospectus 2023-24). The resident is paid a stipend: Kerala Kaumudi reported in July 2026 that it is ₹57,900, ₹58,900 and ₹60,000 a month for years 1 to 3 in government colleges (the 2019 order had set ₹53,000, ₹54,000 and ₹55,000). Confirm the current rate with the college.

How hard is it to get an OBG seat

These are the NMC seat matrix numbers for 2026-27, released on 29 September 2026 and reported by Medical Dialogues on 6 October 2026:

MS Obstetrics and Gynaecology seats, 2026-27Seats

Kerala142
NMC PG seat matrix 2026-27 as reported by Medical Dialogues. The matrix leaves out seats tied to pending applications for more intake or new courses, and the report does not split the Kerala figure into government and private seats.

There are 63,177 MD, MS, diploma, DM and MCh seats in all, and 2,65,960 candidates took NEET-PG 2026. An aggregator site, sartha.in, which is not an official source, lists the 2025 closing rank for MS OBG in Kerala's government state-merit quota at all-India rank 15,803 and in self-financing seats at 66,023. If those numbers are right, a government seat in Kerala needs a rank in the top 6% of the candidates, and the rest of the seats come at a high fee. Check the MCC and CEE Kerala allotment lists for the true figures.

OBG is among the larger branches, so more students get a seat than in small branches. Still, a student who only wants this specialty should plan a second attempt and keep DNB and diploma seats in the list of options.

Gynecology in Kerala: patients, labour room and government hospitals

Almost every birth in Kerala happens in a hospital: 99.8% of births were in institutions in NFHS-5, the National Family Health Survey of 2019-20, according to Down To Earth. In public facilities 37.2% of births were by caesarean section in the same survey (CEDA analysis). For a gynecologist in Kerala, that means regular labour-room and theatre work.

Training is in the labour room and the operation theatre. An OBG resident conducts deliveries under supervision, assists and then performs caesarean sections, and manages emergencies such as bleeding and eclampsia. The number of deliveries a hospital handles shapes the learning, so ask about it when you choose a PG seat.

One major government women's hospital is the Sree Avittom Thirunal (SAT) Hospital, Thiruvananthapuram, a public women's and children's teaching hospital under the Directorate of Medical Education and one of the main teaching hospitals of Government Medical College, Thiruvananthapuram. It was dedicated in 1952 and has obstetrics and gynaecology, paediatrics, reproductive medicine and maternal-fetal medicine departments, according to Wikipedia. We could not find an official figure for its yearly deliveries, so we give none. Every Kerala government medical college has an OBG department; ask each college for its delivery numbers, PG intake and operating sessions before you rank it in counselling.

For a student who studied in Russia, the same advice applies to the MBBS. Obstetrics and gynaecology are taught in the clinical departments of a Russian university, and patients speak Russian. Ask the university about its maternity hospital base and practical hours, and compare the answers across universities using FMGE results and the medical colleges page.

Salary: the official figures

The clearest figure is a Kerala NHM notice. NHM Kollam's walk-in interview notice No. NHM/KLM/133/2026-A3 of 20 August 2026 lists an Obstetrics and Gynaecologist post with an MBBS, a postgraduate degree or diploma in OBG from a recognised university or a DNB in OBG, permanent registration with the Travancore-Cochin Medical Council, an age below 63 and a salary of ₹78,000 a month. It is a contract post, first for three months, and the notice says that it is temporary. We could not find the current Kerala PSC pay scale for a Junior Consultant in OBG and give none; see doctor salary after MBBS from Russia for the government MBBS-level figures. For private hospitals and clinics there is no official figure, and the pay depends on the hospital and the number of deliveries and operations.

Other specialties on the same road

If the student is not sure about OBG, the steps up to NEET-PG are the same for every specialty. Read about the surgical branch in how to become a surgeon, and about three other popular branches in how to become a cardiologist, how to become a neurologist and how to become a dermatologist. How to Become a Gynecologist: Frequently Asked Questions

How many years to become a gynecologist after 12th?

At least 8.5 years with an Indian MBBS: 4.5 years of study, 1 year of internship and 3 years of MS or MD in OBG. With an MBBS from Russia it is at least 10 years, because the 6 years there are followed by FMGE and a 12-month internship in India. Most students take longer because of NEET-PG preparation.

What is the difference between MS OBG, MD OBG, DNB and DGO?

MS and MD OBG are three-year degrees in medical colleges; the title differs by college, but both qualify you as a gynecologist. DNB is a three-year National Board qualification, mostly in large hospitals. DGO is a two-year diploma, and the NMC regulations of 2023 are moving diploma seats towards degree seats.

How much does it cost to become a gynecologist in Kerala?

With a Kerala government MBBS and government PG seat, tuition is about ₹2.8 lakh in all. With a self-financing MBBS seat and a government PG seat it is about ₹38 lakh. A Russian state university outside Moscow costs about ₹14–33 lakh for six years, plus a government PG seat at about ₹1.7 lakh, but living costs abroad come on top.

Can I become a gynecologist after MBBS from Russia?

Yes. You must pass FMGE, complete the 12-month internship in India and register, then take NEET-PG like any MBBS doctor and get an OBG seat. Kerala government PG seats carry nativity and bond conditions, so read the current CEE Kerala prospectus.

Can I become a gynecologist without MBBS or without NEET?

No. Obstetrics and Gynaecology as a specialty of modern medicine needs an MBBS, and an MBBS in India needs NEET-UG. Under the NMC rules, NEET-UG is needed for an MBBS from abroad too. Ayurveda courses such as BAMS lead to a different system of medicine with its own postgraduate degrees.

Can a man become a gynecologist?

Yes. OBG is open to men and women on the same NEET-PG ranks. Some patients prefer a woman doctor, but admission is by rank and not by sex.
