How to Become a Cardiologist After 12th: Course, Years, Cost and Seats

To become a cardiologist in India you need an MBBS, a 3-year MD in General Medicine and a 3-year DM in Cardiology, which takes at least 11.5 years after Class 12 with an Indian MBBS and 13 years with an MBBS from Russia. This guide gives the exams, the years, the fees we could source and the competition at every step, for a student from Kerala.

In short: a cardiologist is a doctor with three degrees one after the other: MBBS, then MD in General Medicine (NEET-PG), then DM in Cardiology (NEET-SS). The shortest time after Class 12 is 5.5 years for an Indian MBBS with internship plus 3 plus 3, which is 11.5 years. With an MBBS from Russia it is 6 years of MBBS and internship there, 1 year of internship in India, 3 years of MD and 3 years of DM, which is 13 years if FMGE and both entrance exams go right first time. Tuition for the two postgraduate degrees is about ₹7.3 lakh in Kerala government colleges and over ₹1 crore in self-financing colleges, at 2023-24 fees. The 2026 NEET-UG qualifying mark is 213/720 for general and EWS, but a cardiology seat is decided by your NEET-PG and NEET-SS ranks.

How to become a cardiologist after 12th: the route

There is no shortcut and no "cardiology course" that replaces these steps. India's medical system builds a cardiologist in layers. Each layer has its own entrance exam, and each is competitive. Here is the order, with the years for a Kerala student who takes an Indian MBBS and for one who studies in Russia.

StepExam or requirementYears: Indian MBBSYears: Russian MBBS
1. Class 12 with Physics, Chemistry, Biology50% marks (40% for reserved categories)00
2. NEET-UG2026 qualifying mark 213/720 for general and EWS00
3. MBBS and internshipIndia: 4.5 years of course plus 12-month internship. Russia: 6 years including the 12-month internship at the same university5.56
4. FMGEOnly for foreign graduates; 150 of 300 marksnot needed0 if passed on the first session
5. Internship in India12 months of supervised CRMIincluded above1
6. NEET-PG, then MD General MedicineRank decides the college33
7. NEET-SS, then DM Cardiology (or DrNB Cardiology)Needs MD or DNB first33
Shortest total after Class 1211.513
Course lengths: NMC CBME curriculum for the Indian MBBS (4.5 years plus 12-month internship); NMC Foreign Medical Graduate Licentiate Regulations 2021 for the Russian route (54 months of study in English plus 12-month internship at the same university, then FMGE and a 12-month internship in India); 3 years each for MD and DM as listed in the SCTIMST DM admission notice. The minimum assumes no failed attempts and no waiting between steps.

Real careers run longer than the minimum. A student rarely finishes an internship on the day an entrance exam is held, and NEET-SS needs the MD to be complete by a cut-off date set each year by NBEMS. A gap of one to two years in total is common, though we found no published figure for it. Plan for 12–15 years after Class 12, which means a fully qualified cardiologist at about 30 to 33 if you start at 17 or 18.

Step by step: what each stage involves

1. Class 12 and NEET-UG

You need Physics, Chemistry and Biology in Class 12 and NEET-UG, whether you want an Indian or a foreign MBBS. The NEET-UG 2026 result came on 16 July 2026 after the 3 May exam was cancelled over a paper leak and re-held on 21 June. The qualifying mark was 213/720 for general and EWS (144 in 2025), 194 for general PwBD and 177 for SC/ST/OBC. Our post-result guide, NEET 2026 result: what next, shows the options for each score band. Qualifying is enough to join a Russian university. It is not enough for a Kerala government seat, and that matters for what comes later, as the cost section shows.

2. MBBS in India or abroad

A seat in Kerala's government colleges costs about ₹23,150 a year in tuition. Self-financing colleges charge about ₹8.1 lakh a year. Russian state universities charge, for the first year, between ₹2.4 lakh and ₹14.1 lakh in tuition in our list; multiply by six for the course and add a reserve for rises. See the MBBS in Russia vs India post and the Kerala vs Russia fee comparison. A cardiologist from a Russian MBBS has the same MD and DM as one from Kerala. The specialist degrees do not record where the MBBS was done.

3. FMGE for Russian graduates

Every Indian with a foreign MBBS must pass FMGE before the internship in India and registration. The pass rate of Russian graduates was about 25% over 2022 to 2025 combined, 18.1% in June 2025 and 30.9% in December 2025. That is why we ask families to pick a university by its FMGE record; see the FMGE pass rate by university. NExT, the exit exam meant to replace FMGE, has no start date, and an NMC draft of 2026 keeps FMGE until NExT begins.

4. Internship in India

The 12-month CRMI is compulsory for Russian graduates after FMGE. It is also the point where NEET-PG becomes possible, because NBEMS wants the internship finished by a cut-off date. For NEET-PG 2026, held on 30 August 2026, that date was 30 September 2026, as reported by Careers360. Read MBBS internship rules from the NMC before you plan the year.

5. NEET-PG and MD General Medicine

A cardiologist normally starts with an MD in General Medicine, a 3-year degree. SCTIMST, for example, lists "MD/DNB in General Medicine or Paediatrics" as the qualification for its DM Cardiology. A broad-speciality seat is allotted by your NEET-PG rank and counselling. In 2026, 2,65,960 candidates appeared for NEET-PG, and the result came on 24 September 2026.

6. NEET-SS and DM Cardiology

After the MD you sit NEET-SS, the super-speciality entrance run by NBEMS, for DM and MCh seats. Reports put NEET-SS 2026 in December 2026, but the dates differ between sources, so wait for the NBEMS notice. Some institutes, among them AIIMS, PGIMER, JIPMER, NIMHANS and SCTIMST, admit DM students through a separate exam, INI-SS. The DM is 3 years long.

The DNB alternative

NBEMS gives a second route. You do a DNB in General Medicine in an accredited hospital, entered through NEET-PG counselling, and then a DrNB in Cardiology through NEET-SS. Kerala PSC notices for Assistant Professor in Cardiology accept DM, DNB or DrNB in Cardiology, but check each notice, because posts and age rules differ. In the MCC matrix for NEET-SS 2025 there were 485 DrNB Cardiology seats.

Cardiology course: what the term covers, and what it does not

People search for "cardiology course" or "cardiologist course" and meet very different things. Only one makes you a cardiologist.

CourseEntryLengthResult
MBBS, MD General Medicine, DM Cardiology (or DrNB)NEET-UG, NEET-PG, NEET-SS11.5 to 13 years in allCardiologist, a registered specialist
BSc Cardiovascular TechnologyClass 12 science, no NEET in most colleges (check each)Usually 3 to 4 yearsTechnologist who runs ECG, echo and cath-lab equipment; not a doctor
Diploma or short certificate in cardiologyVariesMonths to 2 yearsNot a route to specialist registration; ask the state medical council before paying
Searches such as "bsc cardiovascular technology" and "diploma in cardiology" are common, but only the first row is a medical specialist degree. Course lengths for technology programmes vary by university; check the prospectus.

A technology degree is a good career for a student who wants to work in a cath lab without the 12 years. It is a different job. If your child wants to be called Dr, prescribe and perform procedures, the first row is the only answer.

What a cardiologist does, and the sub-specialities

A cardiologist treats heart disease: angina and heart attack, heart failure, rhythm problems, valve disease, high blood pressure and the follow-up of patients after bypass or stents. The DM trains you in all of it, and afterwards many doctors choose a narrower field.

  • Non-invasive and clinical cardiology. Clinic, ward and ICU work, echocardiography, stress tests and heart-failure care.
  • Interventional cardiology. Angiography and angioplasty (stents) in a cath lab.
  • Electrophysiology. Rhythm disorders, pacemakers and defibrillators.
  • Paediatric cardiology. Heart disease in children, usually reached from an MD in Paediatrics. SCTIMST admits paediatricians to its DM Cardiology too.

Be ready for the workload. Cardiology means emergency calls at night, heart attacks that cannot wait for morning, and long hours standing in the cath lab. The SCTIMST resident agreement says residents' hours will not normally exceed twelve continuous hours a day, subject to exigencies of the department. That is the written rule for one institute; do not assume the same in every hospital.

Where to train in Kerala

The most famous address is Sree Chitra Tirunal Institute for Medical Sciences and Technology (SCTIMST) in Thiruvananthapuram, an Institution of National Importance under the Department of Science and Technology. Its DM admission notice lists DM Cardiology with 6 general-quota seats and 3 sponsored seats, 3 years long, and says it takes MD or DNB in General Medicine or Paediatrics. Admission is through the INI-SS exam, not NEET-SS. The notice on sctimst.ac.in is undated, and seat numbers change by session, so read the current INI-SS prospectus.

The Government Medical College, Kottayam, lists 4 DM Cardiology seats on its own course page, again undated. Other Kerala government and private colleges also run DM units, and a private one, Amala Institute in Thrissur, shows 4 DM Cardiology seats in its prospectus table. We did not verify the seat counts of the Thiruvananthapuram, Kozhikode and Ernakulam government colleges, so use the MCC seat matrix for the exact figure in the year you apply.

How much it costs to become a cardiologist

The biggest cost sits in the two postgraduate degrees, not in the MBBS, and it depends on one thing: whether you get a government seat. The official fees we could source are below. They are from 2023-24, the last Kerala government fee order we could trace, and may have been revised.

StageFeeSource and note
MBBS, Kerala government collegeabout ₹23,150 a yearTuition only
MBBS, Kerala self-financing collegeabout ₹8.1 lakh a yearOne college's 2025-26 provisional fee was ₹8,10,175
MBBS, Russian state university₹2.4 lakh to ₹14.1 lakh a yearFirst-year tuition from price orders; see fees in Russia
FMGE (October 2026 session)₹4,650NBEMS bulletin; each attempt
MD, Kerala government college₹57,890 tuition a year₹81,050 first payment with other fees and a refundable deposit (Kerala G.O.(Ms) No. 92/2023, as reported)
DM (PG super-speciality), Kerala government college₹1,84,410 a yearKerala fee order, September 2023, as reported by Onmanorama
MD, DM in a Kerala self-financing college₹17,61,665 a yearSame order; the NRI-quota fee is ₹37,01,600 a year
Sources: Kerala fee order of September 2023 for 2023-24 as reported by Onmanorama (15 September 2023); Kerala G.O.(Ms) No. 92/2023/H&FWD as reported by Medical Dialogues; NBEMS FMGE bulletin October 2026; CEE Kerala; university price orders in our fee data. Tuition only; hostel, mess, books and exam fees are extra. Check the CEE prospectus for 2026-27 (published 7 October 2026) for current figures.

Three simple arithmetic cases for the two postgraduate degrees alone, tuition only, at those fees:

  • Government MD and government DM: 3 × ₹57,890 plus 3 × ₹1,84,410 is ₹1,73,670 plus ₹5,53,230, which is ₹7,26,900.
  • Self-financing MD, government DM: 3 × ₹17,61,665 is ₹52,84,995, plus ₹5,53,230, which is ₹58,38,225.
  • Self-financing MD and DM: 2 × ₹52,84,995, which is ₹1,05,69,990 or about ₹1.06 crore.

Add the MBBS. For a Russian MBBS it is six times the yearly price in the table, plus living costs, flights and the FMGE attempts. For a family that cannot pay ₹50 lakh or more for a postgraduate seat, the plan has to rest on a government seat, and that needs a good rank. A student loan can be sized for the MBBS, but a loan sized for a self-financing PG seat is a very large debt against a salary that begins only after the DM. The money you earn as a resident helps, as the next section shows.

Stipend during MD and DM

PG students in Kerala government colleges are paid a monthly stipend. Kerala Kaumudi reported in July 2026 that the present stipend for the three years of PG is ₹57,900, ₹58,900 and ₹60,000 a month, and that a promised revision had not come. That is ₹21,21,600 over three years, about ₹21.2 lakh. For super-speciality residents the last Kerala order we could trace, G.O.(Ms) No. 101/2019/H&FWD effective 1 July 2019, gave ₹63,000, ₹65,000 and ₹67,000 a month, and the same order is reported to have raised the PG rates to ₹53,000, ₹54,000 and ₹55,000. We did not find a current DM stipend for Kerala, so ask the college. In a self-financing college the stipend is set by the college; do not assume one. SCTIMST pays its residents under its own rules, and its brochure for 2025 shows a junior resident basic pay of ₹61,300 a month plus allowances for its MD in Transfusion Medicine, as an indication of institute-level pay.

How competitive the seats are

Every step narrows. In NEET-PG 2026, 2,65,960 candidates appeared, and reports of the seat total vary from over 50,000 to 86,360 depending on the date and what is counted. Medicine seats in good government colleges go to the top ranks first.

The super-speciality step is tighter. The MCC seat matrix for NEET-SS 2025 listed 542 DM Cardiology seats and 347 DM Neurology seats among 4,312 DM and MCh seats of all kinds. That is the national figure for the whole country, and it includes private colleges where fees run into crores. The MCC special stray round, held in late September 2026 after NBEMS cut the qualifying percentile from the 50th to the 30th, still had 141 DM Cardiology seats vacant. We could not find a breakdown by college or fee, so do not read vacant seats as easy or cheap.

The honest conclusion is that the MBBS decision and the NEET-UG score are only the first filter. A cardiology seat is won again in NEET-PG and again in NEET-SS, and every student, Indian or Russian graduate, competes in the same list.

What a cardiologist earns in Kerala

We found no reliable published figure for private hospital cardiologists, and we do not quote one. What the official documents show:

  • The NHM Kerala notification for Kollam of 17 October 2025 offered ₹78,000 a month, consolidated, for a Specialist Medical Officer in Surgery or Cardiology on contract. A general Medical Officer with MBBS only was offered ₹50,000.
  • Kerala PSC notices for Assistant Professor in Cardiology in medical colleges ask for DM, DNB or DrNB in Cardiology and Kerala medical council registration, and state the pay as "as per UGC norms", with no rupee figure.
  • For comparison, an Assistant Surgeon with only an MBBS is paid on the Kerala scale ₹63,700 to ₹1,23,700.

More on this is in doctor salary after MBBS from Russia. A cardiologist's income comes later than a general doctor's and rises with experience and private practice, but at 2023 fee levels a self-financing DM can take years to repay.

PG abroad, briefly

A graduate from Russia can also train overseas. In the US the route is USMLE Step 1, Step 2 CK and ECFMG certification, then a residency in internal medicine and a cardiology fellowship; the 2026 fee is $695 for Step 1 and for Step 2 CK. In the UK it is the GMC's PLAB tests, which cost £283 for PLAB 1 and £1,036 for PLAB 2 from April 2026 as quoted by study sites, or the UKMLA where the GMC asks for it, then specialty training. Each country judges your medical school separately, so check your university on the ECFMG and GMC lists first. See after MBBS in Russia for the details and fees.

If you choose Russia for the MBBS

  • Choose a university with a consistent FMGE record, because the first gate to cardiology is FMGE, then the Indian internship.
  • Check that the course meets the NMC rules: 54 months in English, the 12-month internship at the same university, a licence to practise in Russia. See the NMC rules for foreign medical graduates.
  • Start NEET-PG study in the final years. The syllabus is the Indian MBBS syllabus.
  • Do not borrow for a self-financing PG seat on the assumption that you will pay it back easily.

Other specialities have shorter or different routes. See how to become a neurologist, a surgeon, a gynecologist and a dermatologist.

How to Become a Cardiologist: Frequently Asked Questions

How do I become a cardiologist after 12th?

Take PCB in Class 12, qualify NEET-UG, complete MBBS, then pass NEET-PG for an MD in General Medicine and NEET-SS for a DM in Cardiology. Each degree has its own entrance rank. A student from Russia adds FMGE and a 12-month internship in India.

How many years does it take to become a cardiologist?

At least 11.5 years after Class 12 with an Indian MBBS: 5.5 for MBBS and internship, 3 for MD, 3 for DM. With a Russian MBBS it is at least 13 years. Real careers take 12 to 15 years because of exam timing and repeat attempts.

Can I become a cardiologist without NEET?

No. NEET-UG is compulsory for MBBS in India and for an Indian student going abroad under NMC rules. A BSc in cardiovascular technology does not need NEET in most colleges, but it makes you a technologist, not a doctor.

What is the cardiologist course fee?

In Kerala government colleges, tuition was ₹57,890 a year for MD and ₹1,84,410 a year for DM in the 2023-24 orders, about ₹7.3 lakh for both degrees. In self-financing colleges the fee was ₹17,61,665 a year, so over ₹1 crore for both. Add the MBBS fee.

Can I become a cardiologist after MBBS from Russia?

Yes. After FMGE, the internship in India and registration, you compete in NEET-PG and NEET-SS like any other doctor. The MD and DM carry no mark of the country of your MBBS. The extra cost is the time and the FMGE risk, which was about 25% pass for Russian graduates in 2022 to 2025.

Is DM Cardiology hard to get?

Yes. The MCC matrix for NEET-SS 2025 had 542 DM Cardiology seats for the whole country, and SCTIMST lists only 6 general-quota seats. Government seats close at high ranks. Some private seats were still vacant in a stray round, but their fees are very high.
