How to Become a Neurologist in India: Years, Cost, Seats and Salary

To become a neurologist you need an MBBS, an MD in General Medicine or Paediatrics and a 3-year DM in Neurology. That is at least 11.5 years after Class 12 with an Indian MBBS and 13 years with an MBBS from Russia. This guide shows the years by age, what each step costs, how few DM Neurology seats exist and what pay we could document in Kerala.

In short: a neurologist is a physician who has completed MBBS, an MD (usually General Medicine) through NEET-PG and a DM in Neurology through NEET-SS or INI-SS. A student who starts MBBS at 18 can be a neurologist at about 29.5 with an Indian MBBS and at 31 with an MBBS from Russia, if nothing goes wrong. The national DM Neurology list had 347 seats in the MCC matrix for NEET-SS 2025, against 542 for cardiology, and SCTIMST in Thiruvananthapuram lists 6 general-quota seats. In a Kerala government college the tuition for MD plus DM is about ₹7.3 lakh in total, and the PG stipend is larger than the tuition. We found no official salary figure specific to neurologists, and say so below.

What a neurologist is, and what is not neurology

A neurologist treats diseases of the brain, spinal cord, nerves and muscles without surgery. Stroke, epilepsy, migraine and other headaches, Parkinson's disease, dementia, multiple sclerosis, neuropathy and myasthenia are the daily work. The route to this title is the same in every Indian state: the title belongs to a doctor who holds a DM or DrNB in Neurology.

Two neighbouring fields are often confused with it. A neurosurgeon operates on the brain and spine and trains through an MS in General Surgery followed by an MCh in Neurosurgery; see how to become a surgeon. A psychiatrist treats mental illness through an MD in Psychiatry, which takes 3 years after MBBS and not 6. A student who likes the brain but wants a shorter road can think about psychiatry; one who likes the operating theatre should read the surgeon page. Only the physician route leads to DM Neurology.

How many years to become a neurologist: the timeline by age

The table assumes a student who joins MBBS at 18 and loses no year: no failed paper, no waiting for a seat, no repeat of an entrance exam. Anything that goes wrong adds to the age, not just to the years.

MilestoneIndian MBBS: ageRussian MBBS: age
Joins MBBS after NEET-UG1818
MBBS and first internship complete23.5 (4.5 + 1 years)24 (6 years, internship included)
FMGE and internship in India donenot needed25 (1 year of CRMI)
MD in General Medicine done26.528
DM Neurology done: neurologist29.531
Course lengths: NMC CBME curriculum (4.5 years plus 12-month internship) for the Indian MBBS; NMC Foreign Medical Graduate Licentiate Regulations 2021 for the Russian route (54 months in English plus 12 months of internship at the same university, FMGE, 12-month internship in India); 3 years each for MD and DM as in the SCTIMST DM admission notice. Age is the start age plus years of study.

That gives 11.5 years from Class 12 for the Indian route and 13 for the Russian. It leaves out the time to pass FMGE (three sessions a year, no limit on attempts), to prepare for NEET-PG and to prepare for NEET-SS. A realistic range is 12 to 15 years. We found no official statistic for the average delay.

Three ways a Kerala family can start: examples with the numbers

These are worked examples, not real students. Fees are the ones we could source (see the cost section for details).

  • A government MBBS seat in Kerala. Tuition is about ₹23,150 a year. A student with a top rank starts here, enters NEET-PG at about 23.5 and is the youngest neurologist of the three. Few get here: Kerala has about 14 qualified NEET-UG candidates for each MBBS seat (see the Kerala vs Russia comparison).
  • A self-financing MBBS seat in Kerala. About ₹8.1 lakh a year in tuition. The route is the same as above, but the family pays the full fee while the PG competition is still ahead.
  • A Russian state university. First-year tuition in our list runs from ₹2.4 lakh to ₹14.1 lakh a year; multiply by six and keep a reserve for rises. The student qualifies NEET-UG at 213/720 or more (the 2026 general and EWS mark) and then needs FMGE. The NEET 2026 result guide shows which score fits which option.

The MD and DM seats are the same for all three. A neurologist does not carry the label of the MBBS university.

Stage by stage: the exams

NEET-UG and MBBS

Class 12 with Physics, Chemistry and Biology and NEET-UG are needed in India and for study abroad. NEET-UG 2026 qualifying marks: 213 general and EWS, 194 general PwBD, 177 SC/ST/OBC (out of 720). The result came on 16 July 2026 after a paper leak forced a re-exam on 21 June.

FMGE and internship, for foreign graduates only

FMGE is a computer-based test of 300 questions with a pass mark of 150. Russian graduates passed at about 25% over 2022 to 2025 combined; the figure was 30.9% in December 2025. The internship in India that follows is 12 months. See after MBBS in Russia and FMGE vs NExT; NExT has no start date and an NMC draft of 2026 keeps FMGE meanwhile.

NEET-PG and the MD

For neurology the feeder degree is an MD or DNB in General Medicine. SCTIMST also admits paediatricians to DM Neurology, which opens a second way in for a doctor who loves children's medicine. NEET-PG 2026 was held on 30 August 2026 with 2,65,960 candidates appearing and the result on 24 September 2026. Your rank decides whether you get Medicine in a government college.

NEET-SS, INI-SS and the DM

DM seats are allotted by MCC counselling on NEET-SS rank, except at institutes like AIIMS, PGIMER, JIPMER, NIMHANS and SCTIMST, which use INI-SS. NEET-SS 2026 is reported for December 2026, but the sources differ between 11–12 and 26–27 December, so check the NBEMS notice. The NEET-SS 2025 round was also reopened after a Supreme Court order led NBEMS to lower the qualifying percentile from 50th to 30th. An alternative is the DrNB in Neurology, which follows a DNB in Medicine; the MCC matrix for 2025 had 261 such seats.

The national figure includes private colleges. We did not find official seat counts for DM Neurology at the other Kerala colleges; use the MCC matrix for the year you apply.

Neurology has roughly two-thirds the seats of cardiology (347 against 542), so there is less room for a rank that is merely good. The 84 vacant seats of the stray round are no bargain by themselves: we found no list of their colleges or fees, and a vacant seat in a college that charges crores is still out of reach for most families. The place to check seats and fees is the MCC seat matrix of the year you apply.

2023-24 fees as reported by Onmanorama (15 September 2023) and Medical Dialogues; deemed-university range from Careers360. Tuition only. Check the CEE Kerala prospectus for 2026-27, published on 7 October 2026, before you rely on any of these.

Over the three years of MD in a government college the tuition is ₹1,73,670. Over the three years of DM it is ₹5,53,230. Together: ₹7,26,900. Put against that the resident's stipend. Kerala Kaumudi reported in July 2026 that PG residents in Kerala government colleges get ₹57,900, ₹58,900 and ₹60,000 a month in years one to three, which comes to ₹21,21,600 over the MD, about 12 times its tuition. The stipend for DM residents is not in a current order we could find; the 2019 order, G.O.(Ms) No. 101/2019/H&FWD, gave ₹63,000, ₹65,000 and ₹67,000 a month for super-speciality residents. So a government seat is a paid training, while a self-financing seat at ₹17,61,665 a year means about ₹52.8 lakh for the MD alone (3 × ₹17,61,665 is ₹52,84,995).

For the MBBS, a Kerala government seat is about ₹23,150 a year, a self-financing seat about ₹8.1 lakh, and Russia is a price per year in the fees table. See the education loan guide before borrowing. The sensible plan is to fund the MBBS and treat a government PG seat as the goal. We do not recommend taking a loan for a ₹50 lakh PG seat on the hope that a neurologist's pay will repay it quickly.

Daily work: what the job asks of you

Neurology is a speciality of history and examination: a long conversation, a careful bedside exam of strength, reflexes and gait, and then tests such as MRI, EEG and nerve conduction studies. Much of the work is chronic: epilepsy, Parkinson's, dementia and migraine clinics where you follow the same patients for years. The urgent part is stroke, where the first hours decide what can be done, so hospitals with stroke units depend on neurologists on call at night.

The SCTIMST resident agreement says working hours will not normally exceed twelve continuous hours a day, subject to department needs. Treat that as one institute's rule, not a rule for every hospital.

Sub-specialities include stroke, epilepsy, movement disorders, neuromuscular disease, headache, neuro-immunology, dementia and neurocritical care. Choose your field after the DM, not before it; the DM covers all of them.

What a neurologist earns: what we could document

We found no official Kerala notification that names a pay for a neurologist, and no reliable salary survey, so we do not quote a private-practice number. The closest documents:

  • The Kerala PSC notification, Category No. 890/2025, for Assistant Professor in Neurology in the Medical Education Service asks for DM, DNB or DrNB in Neurology and registration with the Kerala medical council, and states the pay as "as per UGC norms", with no rupee amount.
  • NHM Kerala notices for Specialist Medical Officers offered ₹78,000 a month, consolidated, for Surgery and Cardiology (Kollam, 17 October 2025) and for other specialities in Ernakulam in 2026. We did not find a notice for Neurology. A general Medical Officer with MBBS only was offered ₹50,000.

Our doctor salary post sets out what we can say for an MBBS doctor. Specialist pay rises with experience and, in Kerala, with private practice, but we cannot give you a figure.

Neurology abroad, in a few lines

The USMLE route needs Step 1, Step 2 CK and ECFMG certification before a US neurology residency. The 2026 fee is $695 for Step 1 and Step 2 CK and $955 for Step 3. For the UK, the GMC's PLAB tests cost £283 and £1,036 from April 2026, as quoted by study sites. Neither country counts your Indian DM automatically, and each judges the Russian university by its own rules. Details are in after MBBS in Russia.

If the plan includes Russia

The same ladder leads to other specialities: cardiologist (the closest, also MD Medicine then DM), gynecologist and dermatologist, which need only an MD or MS.

How to Become a Neurologist: Frequently Asked Questions

How to become a neurologist in India?

Complete MBBS after NEET-UG, then an MD in General Medicine through NEET-PG, then a 3-year DM in Neurology through NEET-SS or INI-SS. A DNB in Medicine followed by a DrNB in Neurology is the other route. A foreign MBBS adds FMGE and a 12-month internship in India.

How many years to become a neurologist?

At least 11.5 years after Class 12 with an Indian MBBS, and 13 with a Russian MBBS. That is 5.5 or 6 years of MBBS and internship, 1 more for the Indian internship after Russia, 3 of MD and 3 of DM. Most students take 12 to 15 years.

Is a neurologist the same as a neurosurgeon?

No. A neurologist is a physician who treats brain and nerve disease with medicines and tests, after an MD and DM. A neurosurgeon operates and trains through MS General Surgery and MCh Neurosurgery, which is a different ladder of the same 6 years after MBBS.

What is the cost of studying neurology in India?

Tuition in Kerala government colleges was ₹57,890 a year for MD and ₹1,84,410 a year for DM, about ₹7.3 lakh for both, and residents are paid a stipend. A self-financing seat was ₹17,61,665 a year, about ₹52.8 lakh for a 3-year course. These are 2023-24 figures.

What is the salary of a neurologist in Kerala?

We found no official figure for neurologists. The Kerala PSC states "as per UGC norms" for Assistant Professor in Neurology, and NHM notices offered ₹78,000 a month for contract specialists in other branches. Private pay depends on the hospital, so ask for the offer in writing.

Can a graduate of a Russian university become a neurologist?

Yes, after FMGE, the Indian internship and registration. Seats in NEET-PG and NEET-SS are open to all registered doctors on rank. DM Neurology had 347 seats in the MCC matrix of NEET-SS 2025, so the rank needed is high whichever MBBS you hold.
